Your Privacy Rights
In 1996, Congress passed the Health Insurance Portability and Accountability Act, also known as HIPAA. This federal law is meant to protect a patient's health care records. HIPAA defines what information may be legally disclosed. Click here for our complete HIPAA privacy notices.
Because we take your privacy rights seriously, we will not ask for your records before your first visit here, as you are not yet a patient of our office. You must request them and bring them to your first appointment and/or send them to us.
If you feel your HIPAA rights have been violated, you have the right to file a complaint with our facility's privacy contact, who is listed with our privacy notices or you may contact the Department of Health & Human Services (Office of Civil Rights) or both.
If you need more information about your HIPAA rights or you have questions about our policies, please call our office and ask for our Privacy Contact Person.
